Office Policies

New Patients

We are currently accepting new patients.

Please check with your insurance plan to make sure that Dr. Nguyen is part of your insurance network. If you have trouble finding out whether or not we are a part of your plan, please feel free to contact us directly with your insurance information and we may be able to help you.

Before your first appointment, please download and fill out Registration Form and Gyn Health Questionnaire. If you are pregnant, please also download and fill out Prenatal Genetic Screen Form in addition to above 2 forms.

You may also fill out forms in the office, but please allow adequate time by coming 15 minutes early. If you are a new patient and arrive 15 minutes late to your appointment, or if you show up on time, but without having your paperwork filled out, we may have to reschedule your appointment.

Prescription Refills

For prescription refills, you can choose one of 3 options:

1)    Contact your pharmacy and ask them to send us refill request electronically through Practice Fusion (This option is preferred)

2)    Contact your pharmacy and ask them to fax us a refill request. Our fax number is 408-520-4575

3)    Email Dr. Nguyen at drnguyen@southbayob.com with your name, DOB, requested medication(s), and name/address of the pharmacy.

We understand that medical information about you and your health is personal. We are committed to protecting medical information about you. We create a record of the health care services you receive from us. We need this record to provide you with quality health care services and to comply with certain legal requirements. Please take time to read our Notice of Privacy Practices. This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review this Notice carefully.

Identity Verification

Per the Federal Trade Commission (FTC) effective August 1, 2009, businesses, including physician practices, will be required to implement identity theft prevention measures. Identity verification will be required at the time patients are checked into the office.  Patients will need to present a photo identification in order to be registered.
